Insurance Information

Lifetime Eyecare works with many insurance carriers and payment plans.

Services and materials from an optometrist may involve both your routine vision discount plan and your medical insurance (if you have medical conditions or symptoms—for example: diabetes, cataracts, glaucoma, eye infection or dry eye).

To see if LIFETIME EYECARE is a panel provider with your vision care plan or your medical insurance carrier, you can consult your HR department at work and call the member telephone number listed on your health insurance ID card.

If you’re looking for routine vision benefits, be sure to ask the name of your routine vision carrier. Your vision plan may be processed through a different company than your medical plan. You should also ask if your plans have a specific doctor panel and who’s on that list. REMEMBER: many plans have OUT-OF-NETWORK REIMBURSEMENT if you choose a doctor not on your panel, but you must ask specifically for that information.
